新发急性缺血性脑卒中患者复发的危险因素
Risk factors of recurrence in patients with new onset acute ischemic stroke
目的 在已建立的新发脑卒中患者队列中,分析急性缺血性脑卒中患者复发的影响因素。 方法 采用巢式病例对照设计,选取聊城市人民医院2017年1月‒2018年9月期间收治的首发急性缺血性脑卒中患者,共纳入1 475例,收集患者基线一般资料和临床生化指标。对出院患者进行36个月随访,监测患者的行为习惯和脑卒中复发事件。采用Lasso回归和多元logistic回归筛选影响复发的主要因素,构建预估复发风险的预测模型;通过受试者操作特征(receiver operating characteristic,ROC)曲线评估模型的预测效力。 结果 共随访到86例(5.83%)复发病例。调整患者一般资料和临床特征后,卒中家族史(OR = 3.780,95% CI:1.154 ~ 12.376)、缺少体力活动(OR = 0.286,95% CI:0.147 ~ 0.557)、高血压(OR = 3.876, 95% CI:1.814 ~ 8.283)和高血糖(OR = 2.358, 95% CI:1.339 ~ 4.153)是急性缺血性脑卒中患者复发的危险因素(P < 0.05);ROC曲线下面积为0.732(95% CI:0.669 ~ 0.796)。 结论 针对急性缺血性脑卒中患者复发的主要危险因素,应采取有效预防措施,包括控制高血压、高血糖,进行适度的体育锻炼等。
Objective To analyze the factors related to the recurrence of stroke patients based on a cohort of acute ischemic stroke patients. Methods A nested case-control design was adopted,a total of 1 475 patients who were admitted to Liaocheng City People's Hospital for first-ever acute ischemic stroke between January 2017 and September 2018 were recruited in this study, of whom general variables and clinical biochemical indices were collected. During 36-month follow-up period, the behavioral habits and recurrence of stroke were recorded. The least absolute shrinkage and selection operator (Lasso) regression and multivariate logistic regression were used to screen the main factors affecting recurrence of stroke. A prediction model for recurrence risk was developed, and the performance of this model was evaluated by receiver operator characteristic (ROC) curve. Results A total of 86 (5.83%) recurrent cases were followed up. After adjusted for general variables and clinical characteristics by multifactorial modeling, family history of stroke (OR = 3.780,95% CI:1.154 - 12.376), lack of physical activity (OR = 0.286, 95% CI:0.147 - 0.557), comorbid hypertension (OR = 3.876, 95% CI:1.814 - 8.283) and diabetes (OR = 2.358, 95% CI:1.339 - 4.153) were risk factors for recurrence of stroke (P < 0.05). The area under the ROC curve was 0.732 (95% CI:0.669 - 0.796). Conclusion Targeting to the main influencing factors for recurrence of stroke, specific measures are recommended, including treatment of comorbid hypertension and hyperglycemia,as well as appropriate physical activities.
